After you leave your employer

After leaving your employer, you will lose your eligibility for membership in the Health Insurance Society and must join the appropriate medical care insurance program based on your individual circumstances.

Returning health insurance cards or Eligibility Verification Certificate

Deadline Within five days from the date of loss of eligibility
Submit documents to Your employer’s health insurance contact (e.g., General Affairs, Labor Management)
Others If a former member of the Hitachi Health Insurance Society undergoes examination or treatment at a medical care institution or other such facility on or after the date of loss of eligibility (i.e., the day after the date of separation) as a Society member, the former insured person will be billed for the portion paid by the Society as unqualified examination or treatment costs.
Promptly return all health insurance cards or Eligibility Verification Certificates after loss of eligibility to prevent unintended use.

Joining the Voluntarily and Continuously Insured Persons Medical Care System

  • Approval notice
  • Request form for automatic debiting of health insurance premiums
    • Required only if requesting automatic debiting as the method of paying health insurance premiums
  • * See the Pre-Application Check Sheet output along with the application form for instructions on obtaining the documents to be attached, precautions, and other information.
Deadline Within 20 days after the day after the date of separation (within 20 days from the date of loss of eligibility)

Joining the Special-Case Retired Persons Medical Care System

Eligibility to join Insured person (employee)

If you are unable to use the application support system

Documents to attach
  • Approval notice
  • Request form for automatic debiting of health insurance premiums
  • Certificate of residence
    • Showing all members (including family members) and omitting permanent domicile and Individual Number
  • A copy of National Pension/Employees’ Pension Insurance annuity certificate
    • (1) If you are already receiving a pension, a copy of the Notice of Determination of Pension / Notice of Change in Payment Amount, Notice of Pension Transfer, or Notice of Revision of Pension Amount is acceptable.
    • (2) If you are currently applying to receive a pension, a copy of the Receipt of Pension Claim Form issued by the pension office is acceptable.
    • (3) If you plan to begin applying to receive a pension in the future, a copy of the Pension Claim Form sent by post from the Japan Pension Service is acceptable.
    • [Note] The document under (3) is not available in a case of accelerated receipt (at age 64 or younger).

  • In principle, if you are aged 64 or younger at the time of retirement, you cannot join the system from the day after the retirement date. (This excludes cases of accelerated pension receipt).
  • If you meet the conditions for joining the Special-Case Retired Persons Medical Care System, you can join the system after losing eligibility as a Voluntarily and Continuously Insured Person of the Hitachi Health Insurance Society or as a member of another health insurance society. To do so, contact the contact point indicated below.
