"Injury and Sickness Allowance" is paid when an insured person unable to earn a salary or other income while unable to work due to a nonoccupational sickness or injury.

Claiming Injury and Sickness Allowance

Documents to attach	Space for doctor’s opinion/space for employer certification Letter of Consent (for Injury and Sickness Allowance) * See the Notes on the Letter of Consent and attach it if applicable. Health Insurance Coverage Survey Form * See the Note on the Survey Form and attach it if applicable. Example (Survey Form (for Injury and Sickness Allowance)) * Documents 4-6 below are required only if you are receiving a disability pension (or disability allowance). Notice of pension remittance (copy) Medical diagnosis from disability pension procedures (copy) * Submit only for the first time. National Pension/Employees’ Pension Insurance pension amendment notice (copy) * Submit when the pension amount is amended.
Deadline	Within two years from the day after each date of inability to work due to medical treatment
Submit documents to	Your employer’s health insurance contact (e.g., General Affairs, Labor Management)
Others	Apply for days of inability to work (including Saturdays, Sundays, and holidays) in monthly payroll units. A separate notification is required in cases of illness or injury attributable to a third party. Contact the contact point indicated below.
