Request for prior consent to provision of personal information to third parties

The Act on the Protection of Personal Information prohibits the provision of personal information to third parties without the consent of the individual concerned. However, the Society handles the following items on the assumption that it has received the implicit, comprehensive consent of members to such provision for the purposes of simplifying procedures for claiming benefits and health activities and improving administrative efficiency. Your understanding and cooperation are appreciated.

1. Insurance benefits and additional benefits

  • In cases eligible for patient cost-sharing reimbursements, high-cost medical care benefits, etc., the Society automatically calculates benefits and pays them to insured persons. Such payments may be conducted via employers.
  • Payment of injury and sickness allowance, the Childbirth and Childcare Lump-sum Grant, etc. may be conducted via employers. In addition, when necessary for payment of benefits, the items indicated on an application form may be checked with employers.

2. Medical care cost information

  • Medical care cost information is sent to insured persons for all members of their household.

3. Name, telephone number, account designated for transfer of funds, etc.

  • Information on name, telephone number, and account designated for transfer of funds may be checked with or provided to employers to ensure that it is up to date.

4. Confirmation of eligibility

  • When a medical care institution or examining institution has inquired to confirm eligibility, the Society may respond by notifying it whether the relevant person is eligible for insurance coverage as necessary for the purpose of insurance benefits or health examinations.

5. Information on state of examination or treatment with medical care institutions

  • As part of its efforts to prevent worsening of lifestyle-related conditions, the Society may provide to employers information on whether members have undergone examination or treatment with medical care institutions.
    Information such as names of medical care institutions or details of examinations or treatments are not provided in such cases.

Exceptions on provision to third parties

The Act on the Protection of Personal Information allows for information to be provided to third parties without the consent of the individuals concerned in the four cases below.

  • As provided for in laws and regulations
    (Ex.: When medical care institutions or health insurance societies send rezepts to a healthcare bill check and payment organization)
  • When necessary to protect human life, health, or property and it would be difficult to obtain the consent of the individual concerned
    (Ex.: Provision to a medical care institution of family contact information for an unconscious individual)
  • When particularly necessary for the purpose of improving public health or promoting the sound rearing of children and it would be difficult to obtain the consent of the individual concerned
    (Ex.: Providing information from health examinations, etc. to public research institutes for epidemiological survey or research)
  • When necessary to cooperate in the performance of duties provided for in laws and regulations by a national or local government agency or a party under contract to such an agency, and obtaining the consent of the individual concerned could impede such performance of duties
    (Ex.: Checking of rezepts in auditing of a health insurance society by a Regional Bureau of Health and Welfare)
