Personal information Q&A

I am absent from work due to illness. What will happen if my superior asks the Health Insurance Society about the name of my condition and related information?

The Health Insurance Society will not provide personal information to a third party (in this case, your superior) without the prior consent of the individual concerned. For this reason, the Health Insurance Society will not provide the name of your condition or related information without your consent.

How is the consent of the individual concerned obtained?

If there is a need to provide personal information to a third party and the consent has not been obtained in advance, in principle the Health Insurance Society will request the consent from the individual concerned in writing.
However, the consent may be requested directly by telephone or other means when unavoidable in an urgent case.

How is the identity of the individual concerned verified in response to a request to disclose personal data?

If the party submitting the request is the individual in question, he or she will be asked to submit a copy of his or her health insurance card and a copy of his or her identification (a driver’s license, for example). If the party submitting the request is an agent, he or she will be asked to submit copies of identification of both the individual and the agent, documentation of the relationship between the individual concerned and the agent, etc.
For more information, refer to “Procedures for the disclosure, correction, and suspension of use of personal data.”

Where should I address inquiries concerning my own personal information?

Please contact the contact point for inquiries concerning personal information by letter or telephone.
When inquiring by telephone, be sure to inform Society staff that your inquiry concerns personal information.
