Can I use the Application Support System to request a Certificate of Loss of Eligibility when leaving my employer?
While still employed, you can make an appointment for issue of the certificate using the Appointment Request for Certificate of Loss of Eligibility (T-401c).
The certificate will be issued after your establishment submits the Notification of Loss of Eligibility to the Health Insurance Society.
