Notification to the Hitachi Health Insurance Society Is Required When a Family Member Becomes Employed
Once a family member (dependent) starts a new job, the health insurance provided by the family member’s employer will take precedence.
However, the family member will not automatically lose eligibility as a dependent under the Hitachi Health Insurance Society.
In addition to the notification under the Income Tax Law, you are required to remove your dependents fromHitachi Health Insurance (notification under the Health Insurance Law).
■Procedure
[SHAREX Portal users]
・Log in to the SHAREX Portal, go to “Application Functions” > “Personal Basic Information ” > “Family Information”, and under “Dependent Information (Health Insurance)”, change “Dependent Status” to “Non-dependent”, then update the information.
・You will be redirected to the Hitachi Health Insurance Society Application Support System.
Follow the on-screen instructions to complete the required input.
・After completing the input, a PDF of the T-012 Notification of Health Insurance Dependent Change (Removal) will be displayed. Please print this form.
・Submit the printed T-012 Notification of Health Insurance Dependent Change (Removal) .
In addition, if the applicable dependents has Health Insurance Eligibility Certificate (postcard format), please submit it together with the form.
* Consult with your employer’s health insurance contact (e.g., General Affairs, Labor Management) if you are unable to complete the procedures in this way (for example, if you cannot access the SHAREX Portal).
[For users who cannot use the SHAREX Portal]
・If you can use the Application Support System of the Hitachi Health Insurance Society, please complete the procedure and then print out the T-012 Notification of Health Insurance Dependent Change (Removal).
・If you cannot use the Application Support System, please print out the application form T-012 Notification of Change in Dependent (Reduction) from the list of application forms on the Hitachi Health Insurance website and fill in the necessary information.
・Please submit the printed or completed Notification of Change in Dependents (Reduction) .
In addition, if the applicable dependent holds a Health Insurance Eligibility Certificate (postcard format), please submit it together with the form.

*Please note
If you have used your Hitachi Health Insurance policy or certificate of eligibility after the date of employment of your family, you will be required to return the medical expenses paid by Hitachi Health Insurance.
In addition, if you delay the procedures for removing yourself from Hitachi Health Insurance coverage, the procedures for returning medical expenses, etc. due to the retroactive expiration of your eligibility may become more complicated.
