   ※The insured person should apply on the web for his or her dependent family members.

  STEP1　Log in to "「MY HEALTH WEB」"
　STEP2　Click on "Apply for Infectious Disease Prevention Points"
                
　STEP3　Select the application type and enter the required information
　STEP4　Upload an image of a receipt or vaccination certificate

　Please prepare in advance, as an image of the receipt or other required

　　documents must be attached when applying.

　　①The name of the vaccine recipient　
    ②The date of vaccination　
    ③Vaccination fee　
    ④The name of the medical care institution
