Has a family member started a new job? [Procedures for family members (dependents) who have started a new job]
Once a family member (dependent) starts a new job, the health insurance provided by the family member’s employer will take precedence.
However, the family member will not automatically lose eligibility as a dependent under the Hitachi Health Insurance Society.
In addition to the notification under the Income Tax Law, you are required to remove your dependents from
Hitachi Health Insurance (notification under the Health Insurance Law).

・On the employee support system (Humanimate21/ESS or SHAREX EXself), register the health insurance dependent status as
“non-dependent” under Basic information of the person Application > Family Information Application.
・You will be redirected to the Hitachi Health Insurance Society Application Support System.
Follow the instructions provided on the screen to complete the application.
・A PDF of the Notification of Health Insurance Dependent Change (Removal) form will be displayed once you complete the application.
Print this form.
・Submit the printed Notification of Health Insurance Dependent Change (Removal) form and the family member’s health insurance card
or eligibility verification certificate, (if you have one issued by Hitachi Health Insurance).
* Consult with your employer’s health insurance contact (e.g., General Affairs, Labor Management) if you are unable to complete the procedures
in this way (for example, if you cannot access the employee support system).
[Others]
・If you can use the Application Support System, please print out the application form “T-012 Notification of Change in Dependents (Reduction)”
after completing the procedures in the application support system.
・If you cannot use the Application Support System, please print out the application form “T-012 Notification of Change in Dependent (Reduction)”
from the list of application forms on the Hitachi Health Insurance website and fill in the necessary information.
・Please submit the printed or completed application form “Notification of Change in Dependents (Reduction)” and your family member's insurance card
or Eligibility verification certificate, (if you have one issued by Hitachi Health Insurance).

If you have used your Hitachi Health Insurance policy or certificate of eligibility after the date of employment of your family, you will be required to return the medical expenses paid by Hitachi Health Insurance.
In addition, if you delay the procedures for removing yourself from Hitachi Health Insurance coverage, the procedures for returning medical expenses, etc. due to the retroactive expiration of your eligibility may become more complicated.
