I became a member of the Hitachi Health Insurance Society as a Voluntarily and Continuously Insured Person after leaving my job. I pay insurance premiums through bank transfer. My name changed after I married; do I need to submit notification of a change in the account to which benefits are remitted? Is notification not required, since I do not pay premiums through automatic debits?
A.
Even if you pay insurance premiums through bank transfer, benefits paid by health insurance in cases in which you incur high medical care costs or in other such cases will be paid by transfer into the bank account registered with the Health Insurance Society. Please submit the Benefit Transfer Account (Change) Form (T-004) if the name on your account has changed.
Contact the Hitachi Health Insurance Society if you pay premiums through automatic debiting; these cases require separate notification.
